Short-stay | Transitional Services

Following a serious illness, accident, or major surgery, you need space to recover, and regain strength. Our communities offer all the necessary resources so that you can get better faster. Our short-stay skilled nursing and transitional care services can be instrumental in helping you get back on your feet in a comfortable and safe environment.

Quality Care—24/7

Nursing care is available 24 hours a day, seven days a week, and our rehabilitation services include physical, occupational, and speech therapies to help patients enhance or regain their independence. Because of our strong relationships with area hospitals and healthcare centers, we are able to promote seamless transitions and care for our patients and residents.

Rehabilitation Therapy Services

We are dedicated to enhancing the lives of those we serve. That’s why we offer our residents and outpatients comprehensive therapy services to help promote a fast recovery and a better quality of life.

Skilled rehab can benefit patients by providing care for a number of situations including:

Physical Therapy

Our experienced team includes licensed physical therapists and licensed therapy assistants who make it their mission to provide residents and outpatients with state-of-the-art therapy regimens while providing encouragement.

Occupational Therapy

Our OT team focuses on helping residents and patients work toward accomplishing their personal goals, reaching milestones, and ultimately, functioning independently. Our program benefits individuals with physical, developmental, or cognitive and perceptual disabilities.

Speech Therapy

Our speech therapists specialize in assisting residents and outpatients who are experiencing speech and language disorders, voice disorders, cognitive communication disorders, dysphagia, and other speech-related issues.

LONG-TERM CARE

Our Long-Term Care program serves those needing 24-hour nursing care for an extended period of time, while also providing a range of physical, speech, and occupational therapies as appropriate. Whatever is needed, whenever it is needed, we’re always here.

Resident Activities

Whether tailored for individuals or designed with a group in mind, daily activities are an important aspect of life at our community. We encourage residents to improve their overall health and make new friends by participating in a variety of activities. We host movie nights, sing-a-longs, and ice cream socials on a regular basis.

Dining & Meal Services

Nicely appointed dining rooms offer residents a fun way to socialize, meet new friends, and interact with others while they enjoy chef-inspired fare that can be tailored to meet dietary needs. Our dining staff is dedicated to making dining and meal services healthy and uplifting.

Worship

We offer ample opportunities for religious and spiritual enrichment including ecumenical worship services and access to spiritual care providers. Everyone is welcome to join us in celebration and worship.

PALLIATIVE AND COMFORT CARE

Available at select Mission Health Communities through local hospice providers, our Palliative Care program serves people with a serious illness who need 24-hour nursing care for an extended period of time. Our care is based on your needs as a person, not your prognosis, and is designed to lessen the symptoms and stress of illness, while improving the quality of life for each patient and their family.

  • Medication management

Comfort Care

When someone we love is approaching the end of life, our main concern is for his or her comfort. In our Community, we believe that everyone has the right to die in peace and with dignity, and according to his or her wishes. We provide end-of-life Hospice Care through local hospice providers. Our goal is to make this precious time as comfortable and peaceful as possible for your family.
Your hospice team may include nurses, a social worker, home health aides, clergy, and volunteers. Spiritual and grief counseling is available, along with extended visiting hours for families.

Specialized Services

Orthopedic Care

Our orthopedic care team utilizes a holistic approach to enhance your quality of life as we accelerate rehabilitative recovery following joint replacement surgery or joint injury. We offer state-of-the-art rehabilitation programming provided by experienced rehab therapists in dedicated gym environments.

Cardiac Management

This program is designed to stabilize and manage cardiac conditions to transition patients back home quickly. Working with local cardiologists, referring physicians, and local hospitals, we focus on the medical and subacute rehabilitation needs of each individual patient by designing targeted treatment plans based on specific cardiac diagnoses.

Pulmonary Management

We utilize an interdisciplinary approach designed to stabilize and manage pulmonary conditions so that patients can transition back home quickly. With a focus on educating you and those who care for you about your condition, we help you set goals for getting back to a healthier lifestyle. We believe that the more you understand your condition, the more effective your treatment will be.

Wound Care Management

Our team includes health care practitioners and nurses who specialize in wound care management, promoting the rapid healing of abrasions, lacerations, contusions, avulsions, diabetic foot ulcers, venous and arterial ulceration, traumatic and surgical wounds.

Intravenous (IV) Therapy

When necessary, IV therapy delivers needed fluids, specialty medications and nutritional supplements to our residents to help prevent unnecessary hospitalizations. Stay with us and receive your care from the comfort of your room.
